Your data will be used solely by Pickle Crew Ph for purposes related to the event you are registering for, including event coordination, communication, registration management, and logistics.
As the Team Captain, you also confirm that you have obtained the consent of your teammates to provide and submit their personal information for the purposes stated above.
All information will be handled responsibly and will not be used for purposes unrelated to the event without the appropriate consent.
Instructions
Only Team Captains are required to fill out this form.
ALL team members MUST create an account on pickleball.com BEFORE registering through this form:
- Accounts on pickleball.com must use their real first and last names.
- Accounts must be synced with their DUPR ID.
Be ready with the following information:
- All members' full names, email addresses, and DUPR IDs
- Each member's shirt size
- Payment proof of the team registration fee of PHP 10,400.00
- Team name and logo
DUPR Teams should have exactly 2 males and 2 females per team.
Categories available:
- DUPR 14 (max. team rating of 14.300, with no player exceeding 4.100)
- DUPR 16 (max. team rating of 16.300, with no player exceeding 4.600)
Your team will be manually entered in the tournament platform's system by the organizers, and eligibility into the category chosen will be determined automatically by the tournament platform. Make sure all your team members' DUPR ratings fall within both the individual and the team limits to avoid tournament platform registration issues.
DUPR Category and Team Name
Make sure your team's total DUPR rating, as well as individual member ratings, fall within the allowed range for the category you're joining.
